- Maintenance

- Equipment inspection
- Cleaning
- Fault finding
- Machinery shutdowns
Typical workplaces include factories, engineering workshops, manufacturing facilities, warehouses, processing plants, construction sites and maintenance departments.
How Is the LT01 Used?
The tag is attached at the relevant lockout or isolation point using the metal eyelet. The required details can then be completed on the reverse of the tag.
The Equipment Locked Out Tag provides a visible warning that the equipment has been locked out and should not be operated while the isolation remains in place.
The tag does not replace the physical lockout device or your workplace’s isolation procedures.
Why Use an Equipment Locked Out Tag?
Lockout tags provide an immediate visual indication that equipment is intentionally out of service.
The LT01 identifies both the status of the equipment and provides space for information about the person responsible for the isolation. This can help workers identify that machinery should not be restarted or operated while maintenance or other work is underway.
LT01 vs LT02 and LT03
LT01 – Equipment Locked Out Tag
Best suited when the main message required is that equipment has been locked out and must not be operated. The reverse includes Date, Name, Company and Remarks fields.

Used where the main requirement is a clear Do Not Operate warning associated with a lockout.
LT03 – Maintenance Department Do Not Operate Tag
Specifically identifies that equipment has been locked out by the maintenance department and includes fields for the responsible person and expected completion information.
Choose the tag whose wording best matches your workplace’s lockout/tagout procedure.
