Do Not Operate Maintenance Department Lockout Tags are designed to provide a clear visual warning that machinery, plant or equipment has been isolated and must not be operated while maintenance, servicing or repair work is being carried out.
This LT03 version is specifically identified for Maintenance Department use, making it easy for workers and contractors to recognise that the equipment has been locked out as part of maintenance activities.
Made from durable PVC, each tag measures 75 mm wide × 146 mm high × 0.5 mm thick and features a reinforced metal eyelet for easy attachment to a suitable lockout padlock, hasp or lockout device.
The tag is double-sided and includes writable fields for recording important information about the isolation.

Front of the Tag
The front provides a prominent DANGER – DO NOT OPERATE warning together with “Maintenance Department” identification.
Writable fields allow the person responsible for the isolation to record:
- Name
- Department
- Expected Completion
This helps other workers identify who is responsible for the lockout and provides useful information about the expected duration of the maintenance work.
Reverse of the Tag
The reverse provides an additional DANGER warning advising that the energy source has been locked out.
It also includes a warning against unauthorised removal of the lock/tag and provides a Remarks section for recording additional information relating to the isolation, maintenance or repair work.
Where Is LT03 Used?
LT03 is particularly suitable for equipment that has been isolated by maintenance personnel or a dedicated maintenance department.
Typical applications include:
- Machinery undergoing maintenance
- Plant servicing and repairs
- Electrical isolation points
- Industrial equipment
- Production machinery
- Switches and isolators
- Valves and energy isolation points
- Equipment temporarily removed from service
- Engineering and workshop maintenance
Common Industries
These Maintenance Department Lockout Tags are suitable for manufacturing, engineering, construction, electrical services, processing facilities, workshops, warehouses, utilities, industrial plants and other workplaces where machinery or equipment needs to be safely isolated for maintenance.
Why Choose the LT03 Maintenance Department Tag?
LT03 differs from a general Do Not Operate tag because it specifically identifies the lockout as being associated with the Maintenance Department.
This makes it particularly useful in workplaces where maintenance teams regularly isolate machinery or equipment for servicing and repair.
The Name, Department and Expected Completion fields also provide a simple way of communicating who is responsible for the isolation and when the work is expected to be completed.
How the Tag Is Used
The tag is attached alongside the appropriate lockout equipment at the isolation point.
The relevant identification fields should be completed before use so other workers can clearly see who is responsible for the lockout.
The tag acts as a visual warning and identification system while the physical lockout device prevents operation of the isolated equipment.
Important Safety Information
A lockout tag provides identification and a visual warning that machinery, plant or an energy source has been isolated.
The tag itself does not physically isolate an energy source and should not be used as a substitute for an appropriate lockout device where physical lockout is required.
Always use lockout tags in accordance with your workplace’s established isolation, lockout/tagout and health and safety procedures.
Related Hazards
Unexpected machinery start-up, electrical energy, moving machinery, stored energy, accidental operation, maintenance hazards and unauthorised interference with isolated equipment.
Helps Support
Workplace lockout/tagout procedures, machinery isolation procedures, maintenance safety systems, equipment servicing procedures and workplace health and safety practices.
